New Patient Information
Please read below the new patient information carefully before you come for your initial consultation.

YOUR APPOINTMENT
Please come 5 - 10 minutes earlier for your appointment to fill out your New Patient Information form. Alternatively, you may fill out the form and bring it with you or email it to us. New Patient Information form can be downloaded by clicking the following PDF icon:
CONSULTATION DURATION
The duration of the initial consultation is approximately 45 min to 1 hour.
RELEVANT DOCUMENTS
Please bring all relevant documents to your initial consultation. e.g. previous blood test, skin prick test results, recent hospital discharge summaries, letters from other specialists, etc.
REFERRAL LETTER AND MEDICARE CARD
Please bring your referral letter from your doctor and Medicare card otherwise you will not be able to claim your medicare rebate. A specialist referral is valid for 3 months and a GP referral is valid for 12 months.
PAYMENT
Payment must be made at the time of consultation. We accept payment by Eftpost, Visa and Mastercard. Please note, cash and AMEX are NOT accepted.
CONSULTATION FEES
Please call us to find out about the cost of consultation, tests and challenges.
$100 CANCELLATION POLICY
Please contact us as early as possible to change your appointment. If you confirm your appointment and cancel within 2 business days of your appointment, you will incur a $100 cancellation fee. If we are unable to confirm your appointment the business day before, your appointment will be released to another patient and we will reschedule your appointment for another date.
STOP ANTIHISTAMINES
Please stop taking antihistamines e.g. Telfast, Clarytine, Zyrtec at least for 3 days before your initial appointment as it may interfere with skin prick test result.
OTHER MEDICATIONS
Please write down a list of medications you are taking and bring it with you to your initial consultation.
You can continue to take other medications that have been prescribed by your doctors. If you have any further questions, please call us for advice.
