DO NOT fill out this form unless you have an appointment with us yet. You will be asked to fill out the form again when you make an appointment if you fill it out first and make your appointment later. You may also attach your referral or any other documents relevant to your appointment.
If you have trouble filling out and signing the form, you may come 5 - 10 minutes early for your appointment to complete a paper form.
CONSULTATION DURATION
The duration of the initial consultation is approximately 45 minutes to 1 hour.
RELEVANT DOCUMENTS
Please bring all relevant documents to your initial consultation. e.g., previous blood test results, skin-prick test results, recent hospital discharge summaries, letters from other specialists, etc.
REFERRAL LETTER AND MEDICARE CARD
Please bring your doctor's referral letter and Medicare card. Without the referral letter, you will not be able to claim your Medicare rebate. A specialist referral is valid for 3 months, and a GP referral is valid for 12 months.
PAYMENT
Payment must be made at the time of consultation. We accept payment by Eftpost, Visa and Mastercard. Please note, cash and AMEX are NOT accepted.
CONSULTATION FEES
Please call us to find out our consultation fees, tests and challenges.
$100 CANCELLATION POLICY
Please contact us as early as possible to change your appointment. If you confirm your appointment and cancel within 2 business days of your appointment, you will incur a $100 cancellation fee. If we are unable to confirm your appointment within the 2 business days prior, your appointment will be released to another patient, and we will message you to reschedule for another date.
STOP ANTIHISTAMINES
Please stop taking antihistamines e.g. Telfast, Clarytine, Zyrtec, cold/flue night tables, Rylatris & Dymista nose spray, at least for 3 days before your initial appointment as it may interfere with skin prick test result.
OTHER MEDICATIONS
Please write a list of the medications you are taking and bring it to your initial consultation.
You can continue taking any other medications prescribed by your doctors. If you have any further questions, please call us for advice.
